Goal

Join the Central West Ontario Health Team [CW OHT] as a patient and family advisor! We need your help to make a meaningful impact. Your insights as a patient, family or caregiver are crucial in shaping the future of a compassionate health care system. The CW OHT is committed to partnering and listening to patients/clients, families and caregivers as equal partners to ensure that changes in the health care system reflect the diverse needs of the communities that it serves. Expanding patient engagement across the CW OHT and its Partners is a key commitment to ensure safe, seamless and people-centred care is realized across the continuum of care delivery.


Description

The purpose of the CW OHT Patient Family Advisory Council [PFAC] is to provide a highly effective and meaningful forum for shared leadership and decision-making amongst patient/client, family, caregivers and CW OHT Partners. As a member of the CW OHT PFAC, you will be encouraged to:

  • Use your individual and collective expertise as patients, clients, families and caregivers with valuable ‘lived and living’ experiences, as well as professional, volunteer and community based skills and abilities
  • Make recommendations based on the needs of the broader community versus individual self interests
  • Make every effort to provide informed advice on CW OHT policy, planning, strategic decision-making and program initiatives

Benefits

This council exemplifies the CW OHT's vision and mission for people-first health with trust and compassion and is an excellent volunteering opportunity for anyone.

At present, volunteers are provided a nominal stipend annually upon completion of each year's involvement, although this is dependent upon funding available. 


Qualifications

  • Receiving or have received care, are or have been a family member/caregiver within Brampton, north Etobicoke, west Woodbridge, Bramalea, or Malton;
  • Comfortable speaking in a group and interacting with others;
  • Able to generate ideas and build consensus on differences of opinion;
  • Able to maintain confidentiality of patient and organizational information; and
  • Not in a position of employment within health care.
  • Involved in the health care system but no medical background is necessary

Time Commitment
This is a Long Term (More than 6 months) commitment
Shifts are available on the following days
 - Tuesday Evening

This is a long Term, 2 year commitment. Members are required to attend the monthly PFAC meetings. These are 1.5 hours and occur once a month, the first Tuesday of every month from 5:00pm - 6:30pm. Outside of these meetings, members also have the opportunity to participate in OHT-led working groups and committees.


Training

There is an Orientation Package that is provided to all new members and support is provided by existing members as required.
