Goal

The Cardiovascular Health Awareness Program (CHAP) raises awareness of Cardiovascular Disease, empowering better self-care and ultimately leading to better quality of life for participants in this program. The CHAP Volunteer will work in this adaptable model of health prevention to measure blood pressure and conduct risk assessments for cardiovascular disease, stroke, and diabetes of seniors in social housing. The CHAP Volunteer is a passionate and enthusiastic individual with a desire to raise awareness, provide health education, and who is trained under the Program to refer participants to social resources in their community.


Description

● Visits seniors in subsidized seniors' housing to conduct the CHAP sessions under the direction of the research nurse supervisor● Welcomes/assists participants as they move through program session(s)● Engages participants for the purpose of conducting cardiovascular, diabestes, and associated risk assessments● Obtains signed consent forms from participants at the start of their engagement● Strict compliance with privacy/confidentiality requirements● Follows/ensures health and safety procedures are complied with● Guided by an electronic database, conducts Cardiovascular and Diabetes Risk Assessments including:- Test/record Blood Pressure (using automated/validated BP testing device)- Physical measurements (waist measurement, weight, etc.)- Instructions to participants on to how to administer a 'Blood Glucose Test' (Diabetes)● Utilizes a tablet computer and other technology to provide information (results) to partner organizations (i.e., Family Health Team, McMaster Department of Family Medicine)● Promotes educational and community opportunities to participants that are specific to their cardiovascular and/or diabetes risk factors (using up-to-date knowledge of community and social programs and services and acts as resource in the community through access to approved information and referral sources)● In the process, provides a beneficial 'social connection' to participants (older adults)● As a volunteer in CHAP:- Collaboration with staff to ensure quality delivery of services and accurate results- Proactive participation in ongoing program evaluation, including input to improve quality- Efficient/effective contact with program coordinator to schedule/keep appointments with participants- Timely filing of status reports, when requested- Promptly brings any concerns to the attention of the program coordinator- Participates in disaster planning, preparation and response, as required- Performs other associated duties, as required

Qualifications

● Understanding of, and adherence to, the CRC fundamental principles and values● Knowledge of and/or involvement in the local community, OR a desire to become involved or learn more (with commitment to build relationships based upon identified needs)● Minimum age - 18● Satisfactory 'Police Vulnerable Sector Check' required● Local travel involved - must have access to reliable personal transportation (motor vehicle or public transit)● Means of contact with participants may vary (volunteer must have reliable, ongoing access to telephone, Internet (with email))● Required to take on-line and face-to-face training (see 'Training Details')● Solid oral/written English skills● Friendly, engaging, conversational (with good listening skills)● Empathy towards, and accommodating of, older adults● Ability to use a computer tablet for data entry● Ability to volunteer as part of a team and independently, with minimal supervision● Organized, reliable (with focus on scheduling/preparedness)● Committed to strictly adhere to CRC policies/procedures● Commitment in favour of diversity and inclusiveness (experience is an asset)THIS ROLE requires a time commitment in excess of 6 MONTHS.

FLEXIBLE, depending upon assignment, with MONTHLY COMMITMENT of 6 HOURSEach visit to a seniors' facility usually lasts around 3 hours (besides travel time)


Training

First Aid Certification (training provided) required.Additional training will be provided to applicants who have achieved the aforesaid Certification and have been accepted into CHAP.
