About the Position
Canadian Red Cross, Ontario Zone Various client residences in Mississauga, Brampton, and Caledon (and from volunteers' homes) Mississauga, Brampton, Caledon, Virtual
● Visits Health TAPESTRY clients in their homes (secondarily connects by telephone, email, Skype) to conduct wellness assessment surveys/questionnaires/check ins (visit duration: 1-2 hours; average monthly total: 6 hours)
● Assists clients in personal health-related goal(s) identification
● Provides a social connection to and for older adults
● Communicates with volunteer organizations, family health team, research team, and/or community based organizations
● Utilizes a tablet computer and other technology to provide information to identified organizations
● Maintains up-to-date knowledge of health programs and services, and acts as a resource in the community through the use of approved information and referral sources
● Collaborates closely with program coordinator to schedule client appointments
● Completes feedback and reports and submits this information to the volunteer coordinator on a regular basis

> Volunteer online and face-to-face training
> Tuberculosis test (reimbursements available)
> Satisfactory 'Police Vulnerable Sector Check'

'Health TAPESTRY volunteers' extend the reach of our inter-professional health care teams. By visiting the homes of our clients, volunteers can help those teams connect better with at-risk populations. Volunteers may do this by bringing information about clients to the inter-professional team and by helping those teams be more responsive to the needs of their clients. Volunteers also play an important role in supporting clients to meet their goals and assist them with staying healthy longer.

● LOCAL TRAVEL REQUIRED (must have access to reliable vehicle, public transit)
● Active listening skills, combined with empathy/patience for older adults
● Ability to volunteer independently with minimal supervision
● Ability to volunteer collaboratively as part of a team
● Strict adherence to Red Cross policies/procedures, especially privacy/confidentiality
● Experience working with a diverse population an asset

+ Standard First Aid Certification required (or willingness to take training)
